PMSBY के तहत लाभ के लिए दावा करने की प्रक्रिया को अनुसरण करें :

• पॉलिसी धारक/नामित व्यक्ति को दावा करने के लिए उस बैंक या बीमा कंपनी से संपर्क करना चाहिए जहां से पॉलिसी खरीदी गई थी।
• दावा पत्र प्राप्त करें और इसे भरें। इसमें नाम, पता, संपर्क जानकारी, अस्पताल का विवरण आदि जैसे विवरण शामिल होंगे।
• आप इस दबा पत्र को जनसुरखा वेबसाइट से प्राप्त कर सकते हैं अथवा निचे दिए गए लिंक से डाउनलोड कर सकते हैं और आप उसे बैंक या नामित बीमा कंपनी से प्राप्त कर सकते हैं।
• पूरा भरा हुआ दावा पत्र दुर्घटना होने के 30 दिनों के भीतर बैंक शाखा में जमा करना होता है।
• फार्म के साथ अन्य सहायक दस्तावेज जैसे कि विकलांगता प्रमाण पत्र या मृत्यु प्रमाण पत्र आदि देना होता हे ।
• बीमा कंपनी विवरण को जाँच करेगी और उसे स्वीकार करेगी।
• स्वीकार्य दावा तब नामांकित व्यक्ति या बीमा धारक के खाते में भेज दिया जाएगा।
• यदि बीमा धारक ने नामांकित व्यक्ति नियुक्त नहीं किया है, तो मृत्यु दावा का भुगतान कानूनी उत्तराधिकारी को किया जाएगा।
